Your day to day as a Social Services Designee As a Social Services
Designee at Crandall Medical Center, you will act as a residents'
advocate, invite family members to care plan meetings, participate
in nursing care conferences, coordinate discharge plans for
residents, conduct and participate in quality assurance studies,
perform duties for resident shopping program and much more.
Engaging with residents and their families will be a key aspect of
your role, fostering a supportive and empathetic atmosphere

Associates degree in Social work or equivalent field Social
Services Certificate preferred Two years of experience working in
