Job Description Description The Palliative Care (PC) Advanced Practice Provider is responsible for providing clinical leadership through comprehensive medical care and support to patients and families living with serious/terminal illness. Will serve primarily as an inpatient consultant, as part of the Palliative Care Consultation Service. Duties as part of the PC interdisciplinary consult team (IDT) may include the following: \n\n \n\t Comprehensive palliative history, perform appropriate physical examination, evaluate diagnostics and interpret findings to provide expertise on symptom management, prognostication, goals of care discussion. \n\t Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care, best practice guidance and research evidences. \n\t Monitor the effectiveness of therapeutic interventions over time, and adjusts recommendations to reflect new information. \n\t Coordinate and collaborate with different IDT members including but not limited to attending, nurses, SW, TCC, hospice teams as is required for optimal patient care. \n\t Contribute to educational development of patient/families regarding end of life care or other matters pertaining to serious illness. \n\t Complete required electronic health record documentation, through clinical notes. \n\t Collaborate with palliative care attending physician as needed for coordination of care. \n\t Participate in quality improvement projects to improve patient care and, palliative & end of life services in the hospital. \n\t Provides staff education through role modeling and in-services to meet identified learning needs and to promote professionalism. \n\t Advance professional development through self-directed learning, continuing education and participation in peer review \n\t Initiates and maintains positive relationships with patients/caregivers. \n\t Initiates and maintains positive relationships with co-workers. \n\t Comply with all applicable laws and standard task as specified by Ohio State Nursing/Medical Board and UH medical bylaws. \n\t Display commitment to the mission of the hospital and its values. \n \n\n Other responsibilities \n\n \n\t Be part of a system for after-hours coverage as the program necessitates in the future \n\t If aligning with professional development plan, engage in outpatient services as the program necessitates in the future \n \n Qualifications Education Requirements \n\n --- Nursing Doctorate or Master of Science in Nursing with emphasis on clinical practice \n\n ---Previous education (e.g. coursework, on-line training modules) in key Palliative Care competencies (communication, symptom management, etc) preferred \n\n License Requirements \n\n --- Current APRN, PA, or CNS license to practice in Ohio \n\n --- Active DEA license \n\n Preferred Qualifications \n\n --- Palliative Care experience \n\n --- Advanced certification in Palliative Care \n\n --- Experience preferred, but will consider new grads with strong MICU background \n
