The facility offers multiple specialty programs including on-site dialysis in collaboration with the Centers for Dialysis Care, a high-quality rehabilitation program and a physician-led cardiology program.:Job Summary:We are looking for dependable, professional, and detail-oriented Licensed Practical Nurses (LPNs) to join our highly respected team of professionals. LPNs will act as compassionate, knowledgeable charge nurses and will provide residents with highest quality care to meet their clinical needs.Job Responsibilities:Candidates will be responsible to:* Provide direct care in accordance with facility policies procedures and standards of clinical care* Possess a strong understanding of long-term care federal and state regulations* Collaborate with physicians, registered nurses and other healthcare professionals to provide the highest quality care to all residents* Administer and accurately document prescribed medications and treatments ordered by the medical staff* Evaluate for assessment, identify, and document resident status and condition changes in a timely and compliant manner* Provide patient education and teaching as needed* Work independently and lead state tested nursing assistants (STNAs) in providing quality care to facility residents based on person-centered care plans* Other duties as requiredEducation and Experience Requirements:* Graduate from an accredited nursing school with an active LPN license required* Excellent organization and communication skills required* Ability to multi-task and remain calm under pressure required* Excellent customer service skills required* CPR Certification Required* Basic computer skills required; experience with Point Click Care preferredWhy Legacy Health Services?:Healthcare is a calling and being part of a family owned and operated company that will invest in YOU will turn that calling into a career.
