APP, you will have a number of opportunities to consider. NFP
offers the opportunity to provide care to underserved and
marginalized populations in a Community Health setting,
contributing to personal and professional satisfaction. Schedule
and Hours: Full-Time: 4-5 days a week Part-Time: 2-3 days a week
Compensation: $100,000 to $121,000 based on Experience, and FTE.
Position Overview: In collaboration with Physicians, the Advanced
Practice Provider (APP) is an Advanced Practice/Certified Nurse
Practitioner (APRN, CNP) or Physician Assistant (PA) that provides
direct patient care, including preventative, primary care, or
chronic care services. Essential Duties: Clinical Practice: Perform
patient assessment, using clinical judgement for the assessment,
diagnosis, or treatment of patients, including prescribing
medication or other treatments. Patient Education: Provide
appropriate education to patients or family members to assist in
the ongoing care and treatment of the patient. Update and modify
plan of care as conditions evolve. Collaborate with community
agencies to plan comprehensive care for patients served.
Compliance: Maintain up to date and accurate patient records.
Participate in annual chart review, biannual prescriptive review,
regular peer review, or other performance improvement activities as
assigned. Other Duties as Assigned Qualifications Nurse
Practitioner Graduate Degree in Nursing (Master’s or Doctorate)
Current and Unrestricted license in State of Ohio as APRN or
equivalent Board Certification (AANP or ANCC) as Nurse Practitioner
in relevant scope of practice Physician Assistant Graduate Degree
(Master’s or Doctorate) from accredited program Current and
Unrestricted licensure in State of Ohio as Physician’s Assistant
NCCPA board Certification Minimum of 2 years as healthcare
practitioner in family medicine, internal medicine or similar
clinical setting preferred. Skills and Knowledge: Demonstrated
clinical competence and understanding. Knowledgeable of public
health policy and practice, particularly related to community
health programs Able to display appropriate cultural competencies
and understanding for a diverse racial, cultural, and ethnic
patient population. Proficient in use of EPIC EHR/EMR and other
