health. Responsibilities A Housing Case Manager contributes to AHF
by utilizing strong experience in Social Services and understanding
the social deterrents of people living with HIV/AIDS and a thorough
understanding of Social Service Programs within Cuyahoga County.
With a caring and knowledgeable focus, you will work with all of
the teams in the Cleveland office so that every patient receives
the support they require and access to available resources. A
Housing Case Manager enhances the reputation of AHF by assisting
patients with their housing and utility needs. This ranges from
assisting a patient with rent arrears to avoid an eviction to
working with patients to find permanent housing and/or providing
utililty assistance. All of this is done while working to keep the
patient in care. You will be a passionate advocate for our top
initiatives. To be successful as a Housing Case Manager it is
crucial to ensure that the AHF core Values and Mission stay top of
mind with all that your do. At AHF we are nimble and able to adapt

collaborative organizational structure where staff are accountable
to multiple leaders. The Housing Case Manager will work as a part
of a close knit team to meet our clients where they are, identify
