Job Description Job Description We’re launching a new nurse-led triage model inside a large senior-focused community with an on-site primary care clinic. The goal is simple but powerful: deliver the right care at the right time—without defaulting to 911 or unnecessary ER visits. As the Triage RN, you’ll be the clinical decision-maker when residents activate their emergency response system, using real-time assessment, judgment, and care coordination to guide next steps. What You’ll Do Respond to emergency response (PERS) alerts and conduct real-time nurse triage via phone/video Rapidly assess symptoms, acuity, medical history, and risk factors Determine appropriate disposition: EMS activation when clinically necessary Same-day clinic visits Home-based urgent visits Telehealth evaluation Coordinate directly with on-site providers and care teams Calm, support, and clearly communicate with seniors during high-stress situations Document assessments and decisions clearly and defensibly Identify recurring issues and contribute to quality improvement efforts Who This Role Is For This role is ideal for an RN who: Thrives in autonomous, judgment-driven nursing Is calm, confident, and decisive under pressure Believes the ER isn’t always the best answer Enjoys building something new and meaningful Required Qualifications Active Ohio RN license 3–5 years of RN experience, ideally in: Emergency Department Urgent Care Triage or nurse advice line Geriatrics or complex chronic care Strong independent clinical judgment Comfort with phone- and video-based assessment EMR and documentation proficiency Preferred Experience Telehealth or remote triage programs PERS, remote monitoring, or virtual care models Community-based or home-based care Medicare / Medicaid populations Value-based or population health settings Why This Role Stands Out No bedside lifting, rounding, or floor assignments Real clinical authority and impact Meaningful work with underserved seniors Direct influence on patient outcomes and ER utilization Opportunity to help shape a scalable care model Competitive compensation for experienced RNs Growth potential as the program expands If you’re an RN who wants to use your judgment, experience, and compassion to make a real difference—this is worth a conversation.
