Role of the Primary Care Physician (PCP) The PCP in our organization is expected to: Take Responsibility for Outcomes: The PCP is accountable for delivering strong clinical care and cost-effective outcomes for a panel of up to 450 patients. They recognize the importance of building trust and influencing patient outcomes through supportive relationships. Act as a Health Coach: Rather than merely addressing illnesses, the PCP helps patients establish and pursue both short- and long-term health goals, regularly following up to ensure progress. Simplify for Action: The PCP prioritizes tasks to make behavior changes more achievable for patients and guides their care teams towards performance objectives. We focus on outcomes and value, regularly measuring metrics such as patient admissions per thousand, appointment utilization (18-21 slots daily), patient experience scores (CGCAHPS), clinical gap closures, and medical cost measures. Each PCP will have specific goals related to these metrics and will collaborate with center and market leadership, as well as care teams, to achieve them. A positive culture is vital in our medical centers, and PCPs are expected to foster an environment of love, accountability, and passion alongside center leadership. The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with managers and medical directors to ensure adherence to guidelines and participate in risk and quality management initiatives, clinical meetings, and other relevant gatherings that promote patient health and organizational objectives. Compliance with departmental goals, performance standards, regulatory requirements, and quality patient care policies is essential. Key Responsibilities: Provide independent care for patients with both acute and chronic conditions, particularly in older adults. Assume full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining involved in the patient's ongoing care. Engage with hospitalists for patients admitted to the hospital, regardless of their affiliation. Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for patients with complex health needs. Lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients visiting the office. For patients unable to visit the office (e.g., in hospitals, SNFs, LTC, or homebound), collaborate with transitional care teams and other available resources. Participate actively in center management and assist in covering for absent providers. Engage in patient recruitment for the center and the organization. Perform additional duties as assigned by management. Knowledge, Skills & Abilities: Competencies for Success Availability and Accessibility: PCPs should be approachable and available for walk-in visits and timely responses to patient communications to build trust. Service Orientation: Deliver care that you would want for your own family members at every patient interaction. Evidence-Based Medicine: Stay informed about evidence-based practices while recognizing the impact of lifestyle and nutrition on health outcomes. Physician Leadership: Continuously develop leadership skills for the benefit of patients, teams, and the organization. Quality: Commit to providing the highest quality of care and work collaboratively with care teams to achieve excellent outcomes. Influence: Effectively and compassionately guide patients and teams towards optimal outcomes. Self-Care: Prioritize personal well-being to ensure the best care for patients, encompassing physical, mental, social, and spiritual health. Proficient in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software. Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment. Fluency in English, both verbal and written; some positions may require proficiency in additional languages. This role requires independent judgment and decision-making. Education and Experience Requirements: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required. Must possess or be eligible for a State Medical License in the relevant state(s). Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required. Maintain board certification through necessary MOC, CME, or retaking exams as required. Current DEA number for controlled substances (Schedules II-V) is necessary. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
