Works with multi-disciplinary team to develop individualized plan of care. Ensure accurate and timely patient documentation. Requirements: Master's Degree in Social Work, Counseling or Marriage and Family Therapy, or Bachelor's Degree in Social Work and relevant licensure (LSW, LISW, LPC, LPCC, MFT, IMFT). State Licensure per state requirements. CPR and CPI Certification within 30 day of employment.
