Description

Responsible for the safe, effective, and cost effective use of medications through the application of specialized knowledge, skills, and professional judgment.

The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patients disease conditions, background, need for understanding and participation in drug therapy in a fiscally responsible manner.

These activities occur in all aspects of pharmacy practice including adult and pediatric inpatient areas, operating room pharmacies, ambulatory oncology and outpatient services ie., Bolwell Pharmacy.

The pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific pharmaceuticals pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and or 4741.

Essential Duties:
Accurate order verification and dispensing.

Minimize order turnaround time.

Medication monitoring and documentation.

Documentation of interventions. Documentation and performance of medication reconciliation.

Inventory control and adherence to Hospital Formulary policies.

Adherence to Hospital and Departmental Policies and Procedures.

Qualifications

Bachelor Degree

R. Ph - Pharmacist - Registered

Previous hospital experience with emphasis on computerized unit dose, sterile products, and patient focused clinical skills preferred.

Good communication skills are essential.

Computer and office equipment, automated compounding systems.
