Job Title: Home Health RN Case ManagerVNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a rewarding opportunity to provide high-quality patient care and make a meaningful difference in the lives of our patients and their families.Job Summary:The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.Responsibilities:

• Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
• Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare providers to ensure seamless care transitions.
• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed.
• Collaborate with our interdisciplinary team to coordinate care, including referrals to community resources and services.
• Document patient progress and outcomes, maintaining accurate and timely records.Requirements:
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Valid driver's license and CPR certification required.What We Offer:
