If so, we'd love to connect with you! We are Mental Health Cooperative (MHC), and we are looking for a Child and Youth CTT Care Manager to join our Clarksville team! You will provide continuous treatment team care management services to consumers with severe and persistent mental illness.

To join our team, you'll need:

• Ability to multi-task consumers' needs to ensure individualized care plan goals are met
• To be an active listener who can identify obstacles, and conceive solutions for the development of individualized care management plans
• Compassionate response to consumers' needs while maintaining healthy boundaries
• Collaborative spirit working with team members to meet consumers' needs
• Ability to transport consumers to appointments in a safe and reliable manner
• Bachelor's Degree in a Behavior Health Field We're especially looking for someone with social work experience within a mental health clinic, school, agency, or hospital environment who is:
