Job DetailsJob Location Pearl Road - Cleveland, OHPosition Type Full TimeEducation Level Graduate Degree -Job Shift 1st ShiftJob Category Nonprofit - Social Services Job Summary: As an expert in their area of oversight, the Manager of Care Coordination and Community Linkages serves as part of an interdependent management team to deliver clinical and administrative oversight in a self-directed manner. The Manager of Care Coordination and Community Linkages is responsible for all departmental functions including compliance and productivity monitoring, departmental goals and targets, external and internal relations, and staff performance. Management function is ultimately to support departmental strategies and ensure staff is supported to provide optimum Program & Service delivery.

    Essential Functions:

    • Provides effective and sufficient workflow direction, clinical supervision, motivation and coaching to staff
    • Proactively addresses and supports team dynamics and relationships
    • Provides prompt follow through and follow up to resolve areas of concern
    • Is an excellent and experienced communicator that can solve problems effectively and think on their feet
    • Manages change and work environment with resiliency
    • Is a master at the macro system level and able to implement micro systems to enhance efficiency and interrelatedness with other Agency departments
    • Specific core competencies, skills, knowledge, and abilities are defined Agency-wide, are provided upon hire and available on the Agency's Intranet

      Required Skills/Abilities:

      • Direct and indirect supervision of direct service staff, providing adequate support to staff for the delivery of Programs and Services
      • Ensure agency standards and department goals are met through analysis of service deliverable data and resulting process improvement strategies and direct staff supervision
      • Implement vision of the agency by coordinating functions of the department and monitoring and providing deliverables
      • Monitor and manage the strategic plan deliverables of the department through contact with Senior Staff and supervision of department staff
      • Manage the department's budget to Agency projections through monitoring of expenses and costs
      • Manage internal and external relationships with other departments, external partners and funders
      • Manage quality by ensuring compliance with local, state and federal laws and funder and governing body requirements and upholds the highest standards of business practices
      • Act as face of RR in community
      • Act as a super user / trainer for staff who use electronic medical record
      • Manage department's employee conduct and performance and timely addresses issues and concerns by providing timely input and direction on disciplinary actions and administrative proceedings
      • Write correspondence to the courts and other external stakeholders regarding client progress

        Provides leadership in program development, analyzing, implementing and monitoring new service and business opportunities

        Education and Experience:

        • Master's degree in relevant field*
        • Additional Licensure as applicable for the department (Dependent on scope of practice and position expectation)

          3 - 5+ years' work experience- already noted in first bullet point

          Minimum Qualifications:

          • LISW or LPCC

            Preferred Qualifications:

            • 3-5 years Behavioral Health Experience
            • Supervisory Experience

              • LISW-S or LPCC-S

                Physical Requirements:

                • Prolonged periods of sitting at a desk and working on a computer and standing, bending and moving.
                • Requires some exposure to communicable diseases or body fluids.
                • Ability to occasionally lift up to 20 pounds.

                  Work Environment:

                  Business hours generally Monday through Friday, however some evening & weekend hours may be necessary to meet all client services / programs. Position may also be required to respond to emergencies (on-call) depending on program / level of care.

                  FLSA Status: Non-exempt

                  Travel: Periodic travel within community and between locations
